How to make an arrangement for the medical examination.
- Reception time for the examination and when the hospital is closed:
- Reception time: 8:30 a.m. - 11:00 a.m.
- In case of emergency, above mentioned time is not applied.
- The hospital is closed on Sat. Sun. National holidays, and Dec.
29th to Jan 3rd.
- Information desk (in front of the Reception desk):
- You can get information about this hospital here.
- Please feel free to ask how to make an application and which clinic
you should go to.
- You should also get a "Mohshikomi-Sho (an application form) here.
- Making an application for the examination:
- If you are new patient, you must go to RECEPTION N0.6 with your
HOKEN-SHO (health insurance card), and filled-out MOHSHIKOMI-SHO
(an application form).
- If you are a returning patient, you must go to RECEPTION No.6
and SHINSATSU-KEN (the consultation ticket).
- If you don't have SHINSATSU-KEN, you only have to show your HOKEN-SHO
and MOHSHIKOMI-SHO to the clerk at RECEPTION No.5.
- Any patients with an appointment should go directory to the reception
desk of each clinic with your SHINSATSU-KEN.
- Reception at the each clinic:
- If you are new patient, you must fill out a MONSHIN-HYOU (the
preliminary inquiry sheet), and give it to the clerk. Please ask
the clerk how to fill out the MONSHIN-HYOU.
- The order you will see the docrtor can be changed depending upon
severity of another patients' condition or symptoms. We will call
your name before the doctors are ready to see you. Please answer
by saying "Hai (Yes)" when your name is called. If you have any
problems, please let us know.
- Making an appointment:
- We ahve no appointment system for new patients. After you see
the doctor, a nurse will help you to make your next appointment.
- If you want to reschedule or cansel the appointment, please call
us at least two days in advance between 2 to 4 pm. Please tell
us youe patient number, so we can make sure who you are and when
you visit us.
- Payment:
- After the examination, the nurse will give you a sheet for KAIKEI
(payment) in a vinyl case which you should take to the reception
No.1.
- Recieving medicine:
- After payment, you will get a numbered card. Wait in front of
the pharmacy until the electric sign tells you your turn, then
you may recieve your madicine,
- In Case of EMERGENCY:
- On weekdays; call 22-3121 and thee the operator to call the clinic
you need.
- On closed days or during the night; call KYUKYU-GAIRAI (the Emergency
Room) through the operater. (22-3121)
- Be sure to tell the clerk or nurse your name, your age, date of
birth, patient number, your latest hospital visit and present
symptom when you call. Come to the hospital with your HOKEN-SHO
(heallth insurance card).
